Documents Re: $750,000 Membership Rights Payment
On August 25, 2008 the waiver and release in connection with obtaining the $750,000 Membership Rights Payment was mailed to NYMEX Class A Members of record as of August 22, 2008. At that time, a labeling error occurred, causing certain names and addresses to be incorrectly printed. The error has been corrected and a subsequent mailing of the materials was sent to each Class A Member on Friday, August 29, 2008, to ensure that each Class A Member receives the materials. If you have not already received a copy of the materials, you should expect delivery shortly. You may also obtain the Notice and Final Waiver and Release Form and Substitute W-9 Form by clicking on the links below: Notice to Class A NYMEX Members Final Waiver and Release Please execute the Final Waiver and Release Form and the Substitute W-9 Form and submit the executed forms (including Medallion Signature Stamp/Guarantee if required) via first class, registered or certified mail to: Computer Share Trust Company, N.A. Corporate Actions Voluntary Offer 250 Royall Street Canton, MA 02021 TO RECEIVE THE MEMBERSHIP RIGHTS PAYMENT, THE ENCLOSED WAIVER AND RELEASE MUST BE PROPERLY COMPLETED, EXECUTED AND DELIVERED TO COMPUTERSHARE TRUST COMPANY, N.A. BY 5:00 P.M., NEW YORK TIME, ON OCTOBER 21, 2008. THE WAIVER AND RELEASE MUST BE EXECUTED WITHOUT ANY CHANGES OR ALTERATIONS. IF YOUR EXECUTED WAIVER AND RELEASE IS NOT RECEIVED BY 5:00 P.M., NEW YORK TIME, ON OCTOBER 21, 2008, YOUR WAIVER AND RELEASE WILL NOT BE ACCEPTED, AND YOU WILL NOT RECEIVE A MEMBERSHIP RIGHTS PAYMENT.
