Members' Group Life and Long Term Disability
Effective April 1 2007, the New York Mercantile Exchange ("NYMEX") will no longer pay the cost of the Members’ Group Life and Long Term Disability ("LTD") policies. Further, the Members’ Benefit Subsidy Program will terminate on that date. Through December 31, 2007 NYMEX has structured a way for the Group Life and LTD plans to continue to be available to you, with the Member paying the cost of the premiums. This option is completely voluntary. In order to maintain these policies, you will be required to do the following. Sign an Acceptance Form and pay the premium due (as set forth below) for the balance of this year for the Life and/or LTD policies. You will subsequently receive the Acceptance Form via mail, e-mail and on the floor. Warren Rosen and his staff will assist NYMEX in this effort. Premium payments must be made to the order of NYMEX and NYMEX will pay the insurance carrier of these policies, i.e., Guardian Life, directly for those accepting coverage. Once received by you, Acceptance Forms and premium payments should be to returned to Warren Rosen, one of his staff members or, alternatively, to Barry Loyal, Vice President of Human Resources, on the 14th Floor of One North End Avenue. The Acceptance Form and a check in the full amount of each of the premiums must be received by NYMEX no later than April 30th in order to be insured past April 1, 2007. Members who do not return the Acceptance Form along with payment for the premiums by April 30, 2007 will be terminated from the Life and LTD policies and not be insured as of April 1, 2007. No exceptions can be made. As mentioned above, the Members' Benefit Subsidy program will terminate as of April 1, 2007. The last subsidy payment made by NYMEX will be for March, 2007. NYMEX has been advised, however, that any benefit plans secured on your behalf by Warren Rosen & Company will not be affected and his presence on the trading floor will continue to service these plans. PREMIUM COSTS FOR LIFE AND LTD POLICIES (April 2007-December 2007)   Group Life Insurance & AD&D $300,000 Life Insurance Coverage – cost per month-$49.50. Payment to continue policy – 4/01/07-12/31/07: $445.50. $35,000 Life Insurance Coverage – (COMEX Option Members only) – cost per month- $5.78 – Payment to continue policy – 4/01/07-12/31/07: $52.02   Group Long Term Disability Insurance Monthly Benefit $4000-cost per month-$93.33-Payment to continue policy – 4/01/07-12/31/07: $839.97 Monthly Benefit $6000 – cost per month-$140-Payment to continue policy – 4/01/07-12/31/07: $1260.
