Subject System Update #10 Cancellations of working orders

CAUSE:

At approximately 10:30 AM CT while working on a production issue, all iLink working orders were inadvertently cancelled via Firmsoft. All customers received cancel acknowledgements on these orders.

RESPONSE:

Senior and development staff immediately generated a list of affected orders. The GCC placed calls to customers and apprised them of the situation. A detailed list of all cancelled orders by firm was created and sent to the firms so they could complete their reconciliation of the affected orders.

RESOLUTION:

Effective immediately, production access to Firmsoft has been eliminated for all development staff.
